Please check the box next to the art form in which you are applying. High school seniors (Grade 12) eligible to complete this application for the State Superintendent Awards for Arts Excellence are those students who, during the course of their high school training, have:

Music: Band Orchestra Vocal 1. been selected and have participated in ALL Oklahoma Music Educators Association's (OMEA) chorus, band, orchestra or jazz ensemble; OR 2. received a superior rating in a vocal or instrumental state solo competition sponsored by the Oklahoma Secondary Schools Activities Association; OR 3. successfully completed the audition process and participated in the Oklahoma Summer Arts Institute; OR 4. participated in a comparable national or state music event.

Piano: 1. been named a district winner in performance for Oklahoma Music Teachers Association Senior Auditions; OR 2. been named a state, regional or national winner of the Music Teachers National Association High School Audition; OR 3. received the Guild High School Diploma; OR 4. received a comparable national or state honor in piano.

Visual Art: 1. successfully completed the jury process and had work exhibited in Young Talent in Oklahoma; OR 2. successfully completed the jury process and had work exhibited in the Scholastic Art or Oklahoma City University Print and Drawing competitions; OR 3. successfully completed the jury process and participated in the Oklahoma Summer Arts Institute; OR 4. successfully completed the jury process and participated in a comparable national or state competition.

Drama/Theatre: 1. qualified for Regionals or placed first, second, or third at the State Speech contest sponsored by the Oklahoma Secondary Schools Activities Association; OR 2. been selected for the All-Star Regional or All-State One-Act Play cast sponsored by the Oklahoma Secondary Schools Activities Association; OR 3. successfully completed the audition process and participated in the Oklahoma Summer Arts Institute Theatre program; OR 4. be a 3Star Member of the International Thespian Society sponsored by the Educational Theatre Association; OR 5. successfully completed the audition process and participated in a comparable national or state program.

Dance 1. exhibited exceptional artistry and technical proficiency in a dance program through high school, private studio, or community program; OR 2. performed in a major dance role or participated in either a dance production, musical, or summer program/workshop; OR 3. successfully completed the jury process and participated in the Oklahoma Summer Arts Institute Dance program; OR 4. successfully completed the audition process and participated in a comparable national program. (Applicants in dance must submit a two- (2) minute videotaped solo dance
